1.     Requesting Your Health Care Records

​You have the right to request a copy of your health care records.


To request records:

  • Submit a written request by contacting your therapist through your client portal; or

  • Phone your therapist to request a Records Request Form/Release of Information

  • Email your therapist with your written request

  • Mail your written request to your therapist at: 8115 Santa Clara Drive, Dallas TX 75218

Please include your full name, date of birth, contact information, and the records requested.
